• A power of attorney & Necessary Information for Filing (This form will be emailed to be filled out with your personal information to start the process).
  

• An original Certified copy of the Certificate of Live Birth. (We will need your BC number only.)

• Beneficiary for your Trust Document (who would you like to leave your assets too?)
